Adult Social Worker, Thurrock Â
£214.13 per day Â
We are seeking a committed and experienced individual to join us as Social Worker in our Preparing for Adulthood Team. This is an agency role, and we are looking for a qualified social worker with experience of working with young people transitioning to adult social care.  You will be working with Young Adults with various learning disabilities and impairments. Â
You will work closely with colleagues in other departments, and agencies to meet the care and support needs, and outcomes for our young adults.Â
You will become part of a stable, award-winning team, where we provide the opportunity for you to grow and develop and join a very innovative and supportive service. Â
You will need to be an excellent communicator and able to work alongside colleagues in Children Services, Health, Education and Voluntary organisations. Â
Job purposeÂ
To provide high quality, professional service to all transitioning young adults who require care support. To carry out strength-based assessments compliant to Care Act 2014Â
To use skills and professional knowledge to ensure people remain safe and well in our communities.Â
